CREPs Logic Model Page 1 of 2
Grant Writing · Program Design · Evaluation
Community Health Education Coordinator, Community Engagement & Consultation Group
Challenge: Limited English proficiency (LEP) and minority residents in Eastern Montgomery County, a community of about 350,000, faced barriers in access to primary care, behavioral health, and support services, compounded by language barriers and stigma.
Approach: Designed a coordinated, one-stop community resource and education program and wrote the $208,000 funding proposal, with SMART goals, a logic model linking activities to outputs and outcomes, and multilingual outreach at libraries, international grocery stores, laundromats, and places of worship.
Result: Submitted for funding. The program is designed to reach 900 residents, deliver 10 health education series in 5 languages, and connect participants to primary care, behavioral health, food, SNAP, and ACA enrollment support.

Community Health Education Coordinator, Community Engagement & Consultation Group
Challenge: Limited English proficient and culturally diverse communities face barriers that make it harder to access healthcare and essential resources, especially when health information and benefit systems aren't designed with them in mind.
Approach: Created and led a weekly, one-hour virtual health education series on Zoom that met community members where they are. Sessions covered physical, mental, and financial wellness, including nutrition, physical activity, stress, sleep, diabetes awareness, patient-centered care, SNAP, and Medicaid. They used interactive presentations, videos, and digital quizzes, with slides translated into Spanish and Haitian Creole. Monthly series were themed around observances such as African Heritage Month and Breast Cancer Awareness Month.
Result: Hosted 8 sessions from September through November 2023, averaging 20 participants per session. Participant feedback directly shaped the curriculum: when members asked for help understanding Medicaid benefits and applying for SNAP, dedicated sessions were added on both. Attendees were connected to Ce-Cg's wraparound support services.
Bridging Language Barriers for Health Equity
Year 1 Work Plan Excerpt
Program Design · Project Management · Evaluation
Sample work plan adapted from MPH capstone, Purdue University
Challenge: In Prince George's and Montgomery Counties, home to nearly two million residents, 13–15% of residents have limited English proficiency, a well-documented driver of unequal health outcomes.
Approach: Designed a three-year implementation plan for a language access program serving 800 adults, covering a language helpline, interpretation and translation services, and cultural competence training for providers. Each phase has clear owners, deliverables, and measurable targets, and evaluation begins with baseline data collected before services launch.
Result: A ready-to-execute roadmap that moves from build and baseline (Year 1) to full service delivery and mid-point evaluation (Year 2) to sustainability, final evaluation, and a replication toolkit for other counties (Year 3).
